Voluntary Life

Employee

  • $10,000 increments
  • Up to 5x annual earnings to max of $1,000,000*

Spouse

  • $5,000 increments
  • Up to the $100,000 max not to exceed 50% of the Employees supplemental coverage*

Children

  • Purchase $10,000 if 6 months to age 19 or 25 (if full time student)
  • Employee coverage must be elected to enroll spouse and/or child and all supplemental life insurance is paid for by you, if elected.

Voluntary Accidental Death & Dismemberment (AD&D)

Employee

  • $10,000 increments
  • Up to 5x annual earnings to max of $1,000,000*

Spouse

  • 100% employee amount of coverage up to $250,000
    Child(ren) Only
  • $5,000 increments amount up to $25,000

Voluntary Accident

This coverage pays a lump sum if you suffer a covered injury as the result of an accident.
